Note: नियमपुस्तिका के नियम सं.क्र. 10.7 और 10.3 के अनुसार, ऐसे उम्मीदवारों का विवाह महिला के लिए 18 वर्ष और पुरुष के लिए 21 वर्ष से पहले विवाह हुआ हो, या तीसरे बच्चे का जन्म 26.01.2001 के बाद हुआ हो (जुड़वां बच्चों को छोड़कर), वे उम्मीदवार इस आवेदन पत्र को भरने के लिए पात्र नहीं हैं, यदि उम्मीदवार फिर भी इन नियमों को अनदेखा करके इस आवेदन पत्र को भरते हैं, और विभाग के द्वारा खारिज कर दिये जाते है, तो इसके लिए उम्मीदवार स्वयं जिम्मेदार होंगे।.

1.High School Examination certificate
2.Original HSSC Marksheet/certificate.
3.Certificate of relevant degree with each semester’s mark sheet, from a recognized university.
4.Caste certificate issued by competent authority of MP Govt. in prescribed format.
5.MP Domicile certificate.
6.Candidates serving in government / semi government / public / private sector should submit N.O.C. from the employer.
7.Photo identity card (Passport / Driving license / Voter ID / Bank pass book) and 2 Recent Passport size photograph
8.Proof of permanent address.
9.Marksheet of diploma in computer.
10.CPCT score card.
